What do I need for the online application?
For each person: name, email, phone number, date of birth, sex and any medications taken. For a family application, the same details for each family member.
What else helps when we talk?
The doctors and hospitals you want to keep, any prescriptions, a rough monthly budget, and your current plan if you have one.
Is my information safe?
Your application is stored only in this site’s database and is read only by Michael after he signs in. The privacy policy explains the details.
What happens after I apply?
Michael reviews it, then calls or emails you to go over your options. Nothing is purchased until you choose a plan.
Can I apply for my whole family at once?
Yes. Choose the family application and add each person.
